The International Association for Six Sigma Certification (IASSC) is a Professional Association dedicated to growing and enhancing the standards within the Lean Six Sigma Community. IASSC is an independent third-party certification body. IASSC does not provide Lean Six Sigma related training, mentoring, coaching or consulting services. IASSC exclusively facilitates and delivers centralized universal Lean Six Sigma Certification Standards testing and Lean Six Sigma Accreditations. These are based on established, recognized and published criteria, none of which are dependent upon the use or engagement with any specific trainer(s), training provider(s), consultant(s), proprietary training content, products or software or any specific organization or company offering Lean Six Sigma or any other services.

Kentucky Commission on Proprietary Education

The Kentucky Commission for Proprietary Education was established by action of the 2012 Kentucky General Assembly; it replaced the Board of Proprietary Education. The commission is charged with licensing and regulating all proprietary schools, doing business in Kentucky, other than those that offer a four year bachelor’s degree.

Policies & Procedures

General Policies - Training & Exams

Enrollment Date Policy: Enrollment at Altitude Career Skills Institute is considered “Rolling.”  Candidates will be given access to their course within 24-hours of purchasing the course online unless otherwise specified.  Start dates can be changed by emailing info@projectaltitude.us or by making a note during enrollment.  Candidates will have six (6) months to complete the training program and 1-year to complete exams from the start date unless otherwise noted.  Access to the courses on our platform have an expiration date of 6-months. Candidates can extend this on a month to month basis. Tuition & fees paid to extend courses will be credited to the candidate should they choose to enroll in another program. 

Candidates should familiarize themselves with cancellation, no-show, and refund policies prior to enrolling. Additionally, candidates should review: Training & Exam Delivery Policies & Procedures section II for requirements regarding enrollment criteria.

Cancel & Reschedule Policy: Canceling your scheduled exam within 48-hours prior to your exam date will incur a cancellation/reschedule fee of $245. To cancel your scheduled exam and reschedule without incurring a fee, you must submit a written request (emails are valid) to info@projectaltitude.us. at least 72-hours prior to your exam date. Canceling the same day is considered a no-show (see policy below). 

Canceling Course Enrollment: Canceling your scheduled course within 48-hours prior to your start date will incur a cancellation/reschedule fee of $245. To cancel your training and reschedule without incurring a fee, you must submit a written request (emails are valid) to info@projectaltitude.us. at least 72-hours prior to your start date. Canceling the same day or later is considered a no-show (see policy below). 

Exam No-show Policy: An Exam no-show means that a candidate failed to join their scheduled exam video conference within 15 minutes of the start time. If you are a no-show for a scheduled exam, you must pay a no-show fee of $245 prior to being permitted to reschedule an exam. 

Training No-show Policy.  A Training no-show means that a candidate failed to make progress in their course as described below. If you are a no-show for training, you must pay a no-show fee of $245 prior to being permitted access back  into your course room.

Absences & Course Progress Policies:  Training at Altitude Career Skills Institute is self-paced and online. Although the training is online and self-paced, candidates are required to make progress over the six (6) month period they have access to the course.  Candidates must make 15% progress each month until 90% of the course training is complete. A candidate does not have to progressively make 15% progress; rather they should strive to meet the schedule displayed below.  If you are unable to meet the schedule, you can coordinate with an instructor a plan to meet your goals on time or extend your course.  Failure to meet the progress requirements for one month will result in an “absence” for the month and a warning from Altitude Career Skills Institute.  To get back on track, a candidate must simply meet the next month's progress requirements.  Failure to meet the progress requirements for two months will result in a second “absence” and Altitude Career Skills Institute will unenroll you from the program and you will incur a “Training No-Show,” with all associated costs.

Required Candidate Attendance Progress %. Month one, (15%); Month two, (30% ); Month three (45%); Month four, (60%); Month five, (75%) Month six (90%).

Veteran Affairs Attendance Policy: Training at Altitude Career Skills Institute is self-paced and online. Although the training is online and self-paced, candidates are required to make progress over the six (6) month period they have access to the course. Candidates must make 15% progress each month until 90% of the course training is complete. A candidate does not have to progressively make 15% progress; rather they should strive to meet the schedule displayed below. If you are unable to meet the schedule, you can coordinate with an instructor a plan to meet your goals on time or extend your course. Failure to meet the progress requirements for one month will result in an “absence” for the month and a warning from Altitude Career Skills Institute. To get back on track, a candidate must simply meet the next month's progress requirements. Failure to meet the progress requirements for two months will result in a second “absence” and Altitude Career Skills Institute will unenroll you from the program and you will incur a “Training No-Show,” with all associated costs. In order to show that the cause of unsatisfactory attendance has been removed, students must show good attendance (as defined) for one calendar month after being terminated for unsatisfactory attendance.After such time, the student may be recertified for VA education benefits. The student’s attendance record will be retained in the veteran’s file for VA and SAA audit purposes. 

Academic Progress for VA Students. Students receiving VA educational benefits must receive a "P" or pass for courses they are enrolled. A VA student who receives a F or I in more 33% of courses or greater will be placed on academic probation for a maximum of two consecutive terms of enrollment. If the VA student’s CGPA still includes and score of F or I for greater than 33% of courses at the end of the second consecutive term of probation, the student’s VA educational benefits will be terminated.

A VA student terminated from VA educational benefits due to unsatisfactory progress may petition the school to be recertified after attaining a CGPA or percentage of "P."

Transcripts and Transcript Requests Policy: Candidates who complete 90% or greater of their online training courses will be granted a certificate of completion for their training.  Candidates who complete their exams with a passing score will receive a certification from the credentialing body with whom they tested and a certification from Altitude Career Skills Institute. Some funding sources require proof of completion.  In these circumstances, Altitude Career Skills Institute will also send a copy of the transcript to the funding source and/or program.  A third party may verify a credential on our website; however, a pdf certificate or certification will only be sent to a third party IF requested by the candidate who earned it. These requests can be sent to info@projectaltitude.us or by using our transfer request form Altitude Career Skills Institute will send pdf transcripts at no cost to the candidate. 

Grading Policy. Altitude Career Skills Institute is an educational institution that emphasizes the importance of acquiring practical skills for success in the workplace. As part of its commitment to ensuring that students meet the required standards, the Institute has implemented a three-tier grading system that includes "pass," "fail," and "incomplete." This system has been designed to provide clear and objective feedback to students on their progress in the course and to help them identify areas where they need to improve. By adhering to this grading policy, Altitude Career Skills Institute aims to promote academic excellence and to equip students with the knowledge and skills needed to achieve their career goals.

Pass: This grade is given to students who have met the requirements of the course and have demonstrated sufficient understanding of the material. Students who receive a "pass" have completed all the necessary assignments, exams, and other requirements of the course to a satisfactory level.

Fail: This grade is given to students who have not met the requirements of the course and have not demonstrated sufficient understanding of the material. Students who receive a "fail" have not completed all the necessary assignments, exams, and other requirements of the course to a satisfactory level.

Incomplete: This grade is given to students who have not completed all the necessary requirements of the course by the end of the term due to circumstances beyond their control (e.g. illness, family emergency, etc.). Students who receive an "incomplete" must complete the missing coursework by a designated deadline in order to receive a final grade of "pass" or "fail."

Failed Exam Policy:  The re-examination fee for a full exam is 65% of the applicable certification exam cost. Altitude Career Skills Institute offers TAKE2 for all exams.  TAKE2 is a free exam retake after the candidate’s first failed exam.

Withdrawal, Dismissal & Reentry Policies: 

Withdrawal refers to the situation where a candidate drops a course or courses. Students may withdraw from a course via the web up 72-hours prior to gaining access to their course room.  Candidates who withdraw from course prior to the course opening will have no grade assignment and no record of having attempted the course, nor will any transcript be kept.  Candidates who withdraw after the 72-hours will have a certificate of withdrawal kept on record annotating progress made. Withdrawals made within or after the 72-hour mark are subject to respective fees except in extenuating circumstances.

Dismissal refers to the situation where a candidate is dropped from a course or courses for reasons otherwise listed between the candidate and Altitude Career Skills Institute.  Students can be dismissed for violations of academic integrity and failure to make progress.  Please see respective policies for clarification.

Reentry refers to the situation where a candidate has withdrawn from or is dismissed and desires to reenter the program.  Re Entry is permitted for students who withdraw prior to their withdrawal date and/or withdraw from the course after the 72-hour mark and remain in good standing with the organization (that is to say the candidate owes no fines or fees).  Reentry for dismissed candidates is on a case-by-case basis.  Please contact info@projectaltitude.us if you are attempting reentry after a dismissal.  Altitude Career Skills Institute will do its best to ensure candidates who reenter the program, receive credit for work completed prior to withdrawal or dismissal.

Refund Policy: Requests for refunds must be submitted to info@altitudecsi.org Refund requests submitted less than 72-hours prior to your scheduled exam or training will incur a $475 processing fee that must be paid prior to the refund being issued. The registration fee will only be refunded if less than 10% of the course has been completed. Completion percentage can be viewed on the Course Progress page from within the course. Refunds will not be granted after a candidate has had course access for seven (7) or more days. The purchase of all exam vouchers is final. Exam vouchers are non-refundable and non-transferable. In purchasing an exam voucher, candidates accept that they may not request a refund once the exam voucher has been issued.

Veteran Affairs Refund Policy: The refund of the unused portion of tuition, fees, and other charges for veterans or eligible persons who fail to enter a course or withdraw or discontinue prior to completion will be made for all amounts paid which exceed the approximate pro-rata portion of the total charges that the length of the completed portion of the course bears to the total length of the course. The proration will be

determined on the ratio of the number of days or hours of instruction completed by the student to the total number of instructional days or hours in the course. The school may retain a registration fee of no more than $10, a breakage fee for no more than the exact

amount of breakage, and fee for consumable supplies for no more than the amount of supplies actually consumed – 38 CFR 21.4255

Credit & Course Transfer Policy: Transfer credit for training courses is not required at Altitude Career Skills Institute.  Some courses do have prerequisites. We give favorable consideration to courses that could appropriately be offered by Altitude Career Skills Institute, whether or not equivalent courses currently are offered. Admissions reserves the right to accept or reject certifications earned at other institutions governed by different credentialing bodies. In general, it is policy to accept certifications earned at institutions fully accredited by their respective credentialing bodies, provided that such certifications have been earned to standard. Exceptions are noted under Notable Restrictions on Transferred Certifications.  If some certifications do not transfer and the candidate questions a decision, the candidate can appeal directly to the Senior Vice President for Academic Affairs at Info@projectaltitude.us. Once a verdict on an appeal for the transferred certification is made, there will be no further appeals.

Notable Restrictions on Transferred Certifications: Altitude Career Skills Institute reserves the right to deny credit for courses that are not compatible with its accredited programs. 

Certifications considered below associate level certification.

Repeated certifications or certifications with duplicate subject content

Coursework earned at an institution that did not hold at least candidacy status with its accrediting association when the certification was completed.

Accommodations Policy: Requests for exam modification accommodations due to disability, handicap, or other conditions will be considered with supporting documentation. Submit requests to info@projectaltitude.us

Complaints and Appeals Policy: Students must address their concerns about this school or any of its educational programs by following the grievance process outlined in the school’s catalog. If you were not provided with this information, please inform school management. Students dissatisfied with this school’s response to their complaint or who are not able to file a complaint with the school, can file a formal complaint with Kentucky Commission on Proprietary Education, as well as with other relevant agencies or accreditors, if applicable. 

All certification related complaints and appeals must be reported in writing to info@projectaltitude.us. within 30 days of the event or incident. We will review and validate the complaint in a timely manner, and you will be notified of the outcome. The record of the complaint or appeal, including any actions taken to resolve the complaint, will be confidential. Additionally, you will be given the opportunity to discuss the course via a feedback form at the end of the course. All candidate feedback documents are reviewed.

To file a complaint with the Kentucky Commission on Proprietary Education, a complaint shall be in writing and shall be filed on Form PE-24, 2017 Form to File a Complaint, accompanied, if applicable, by Form PE-25, Authorization for Release of Student Records. The form may be mailed to the following address: Kentucky Commission on Proprietary Education, 500 Mero Street, 4th floor, Frankfort, Kentucky 40601. The forms can be found on the website at http://www.kcpe.ky.gov/.

Surveys: All candidates will receive a brief electronic survey within 5 days after an examination in order to rate their examination experiences. Responses to the survey will in no way impact exam results. Responses to the survey, however, will be shared with the Altitude Career Skills Institute in order to improve the certification process for future candidates.

Kentucky Student Protection Fund: Pursuant to KRS 165A.450 All licensed schools, resident and nonresident, shall be required to contribute to a student protection fund. The fund shall be used reimburse eligible Kentucky students, to pay off debts, including refunds to students enrolled or on leave of absence by not being enrolled for one (1) academic year or less from the school at the time of the closing, incurred due to the closing of a school, discontinuance of a program, loss of license, or loss of accreditation by a school or program. 

Process for Filing a Claim Against the Student Protection Fund: To file a claim against the Kentucky Student Protection Fund, each person filing must submit a signed and completed Form for Claims Against the Student Protection Fund, Form PE-38, 2017 and provide the requested information to the following address:  Kentucky Commission on Proprietary Education, 500 Mero Street, 4th Floor, Frankfort, KY 40601.  The form can be found on the website at http://www.kcpe.ky.gov/. 

Filing a Complaint with the Kentucky Commission on Proprietary Education: To file a complaint with the Kentucky Commission on Proprietary Education, a complaint shall be in writing and shall be filed on Form PE-24, 2017 Form to File a Complaint, accompanied, if applicable, by Form PE-25, Authorization for Release of Student Records. Other requirements not listed: Altitude Career Skills Institute is a proud member of many credentialing bodies, Accredited Training Organizations and Accredited Training Partners. Altitude Career Skills Institute will make required amendments and addendums to its policies to ensure it is within compliance of its ATPs/ATO/CBs. It is the responsibility of the candidate to review all polices and quality management systems of the credentialing body for which they want to certify as specifics may not be listed in this document; however, these specific requirements are in place.

Altitude Career Skills Institute will make required amendments and addendums to its policies to ensure it is within compliance of its ATPs/ATO/CBs. It is the responsibility of the candidate to review all polices and quality management systems of the credentialing body for which they want to certify as specifics may not be listed in this document; however, these specific requirements are in place.

Impartiality Policy

Impartiality Policy Statement: The Altitude Career Skills Institute Senior Leadership Team ensures that Altitude Career Skills Institute is operated in a manner that safeguards objectivity and impartiality in delivering certification services in a non-discriminatory manner.

Subject to limitations in this document, Altitude Career Skills Institute shall make its services available to all applicants whose application for certification meets Altitude Career Skills Institute’s and associated accreditors publicly available criteria for certifications offered. Certification shall not be conditional upon membership of any association or group, nor is certification conditional upon training service providers. Certification services may be restricted without discrimination where there is an unacceptable risk to Altitude Career Skills Institute, such as (but not limited to) evidence of fraudulent behavior, concealment of information, or providing false information in an application or in an assessment process.

All Altitude Career Skills Institute personnel involved in the delivery of certification services shall act objectively and be free from any undue commercial, financial or other pressures that could compromise impartiality, and are required to disclose potential conflicts of interest.

Altitude Career Skills Institute shall not offer or provide any service that impacts its ability to be impartial. Altitude Career Skills Institute does not provide consultancy, or any other services that pose a risk to impartiality. Altitude Career Skills Institute shall not suggest that certification would be simpler, easier, faster, or less expensive if any specified person or consultancy was used.

Certification decisions shall be made by persons other than those who conducted the assessment.

Altitude Career Skills Institute monitors conformance to this Impartiality Policy through administration of its management system, including its process for managing risks.
